The Clinical Practice Model Resource Center is committed to co-creating healthy, integrated work cultures that positively influence the health and health care of this humanity. We are the connecting point for a large group of people across the continent that we teach and learn with on a daily basis. We are in the field with those who provide health care or support the delivery of health service across the continuum of care. To date, over 100 healthcare settings throughout the United States and Canada have implemented fundamental components of the CPM Framework and have shared many success stories as a result. We also work with people from the community, representing education; business and non-profit sectors, which are learning, developing, and providing diverse services that influence the health of society.
