		<description>It is unfortunate that you/your family and numerous others experience the frustrations and negative quality outcomes from lack of initerdisciplinary integration. My mother recently experienced at least 12 people asking her to tell her story in the ED prior to admission and then again upon admission. We also experienced the benefits of Professional Exchange Report (PER)and the negative impact when there was no PER when my dad had a Stroke with Aphasia.  When there was no PER and no interdisciplinary Plan of Care, we felt that we needed to be present at change of shift and care providers. It was exhausting and very frustrating to see a lack of syncrony within the team and with our beloved family members. When the attributes of Qualtiy were not present, the experience, care and outcomes were suboptimal and we experienced a lack of trust.  The care settings that we were in were recipients of national quality awards but the lack of interdisciplinary documentation and practice did not create a quality patient/family experience.</description>
