Co-Creating an Integrated Health Care System: Executive Update


It is not the need for integration that is misunderstood. It is the understanding of the multifaceted process on how to make it happen beyond the operations level to the point of care. This workshop is designed for health care administrators, executives and managers who are committed to create an integrated health care system. The session will provide an intimate look at the fundamental work necessary to achieve integration at the Point of Care. Point of Care is where the interdisciplinary teams meet to provide the service. This session will not only identify the complex clinical issues surrounding integration but will show the critical knowledge, skills and structures necessary to be successful. It is not a theoretical overview but a summary of the lessons learned from being in the field with multiple settings and discipline across the continent who are actually doing the work. The desired list of fiscal and clinical outcomes of an integrated health care system will not be realized until there is integration at the point of care. The participants will be able to:
  • Identify the key leverage points of an integrated system at the point of care.
  • State the desired outcomes of point of care integration.
  • Articulate the fundamental work necessary to achieve point of care integration.
  • Delineate the knowledge, structures and skills necessary for point of care integration.
  • Correlate the fiscal and clinical outcomes to the essential foundational work.
